How to apply for a certificate
All the appropriate sections of the application form must be completed
in full.
Incomplete applications or those with insufficient payment will be returned.
Documents can be issued only if the birth, marriage or death occurred in Manitoba.
Certificates contain information extracted from the original registration filed at
our office.
Birth certificates are issued in the applicant's birth name, unless a legal change of name has occurred.
If you have changed your name upon marriage, please provide your name before your first marriage.
THE VITAL STATISTICS AGENCY STRONGLY RECOMMENDS THAT BIRTH CERTIFICATES BE ORDERED WITH THE NAME(S) OF THE PARENT(S).
In Person
Counter service is available at 254 Portage Avenue, Winnipeg.
The
building is located at the SW corner of Portage Avenue and Garry Street.
The Agency
is open Monday through Friday, from 8:30 A.M. to 4:30 P.M.
Available payment methods
are cash, cheque, money order, debit, Visa or MasterCard.
Please note personal
cheques, unless certified, will NOT be accepted for 24 hour RUSH service.

For security reasons, certificates cannot be purchased online at this time.
